The following contracts will assist teachers in the development of their courses and have been designed to improve communication between students, parents and teachers.  They require Adobe® Reader® 6.0 which is free software that lets you view and print Adobe Portable Document Format (PDF) files on a variety of devices and operating systems.

Student/Parent/Teacher Agreement Contract

An agreement form detailing the specific roles and expectations of each participant in an on-line course.

 

Image Usage Agreement Contract

This form should be used if student work or pictures will be placed on the internet.

 

On-line Student Questionnaire

This is a preliminary screening tool intended to inform teachers about potential on-line students. This should be accompanied by a student/teacher interview and is designed to define expectations as well as make sure that the students will be successful in an on-line environment.
