The Student Information
Form will be available at your school site for your
review and signature. At this time we are still
required to have a manual signature on all forms.
You should have received a letter with login and
password information to allow you to make updates to
student information electronically. You
may access the online student information update website

forms listed below. When you sign the Signature Verification
form you are acknowledging that you have read all
required forms.

below, print and sign.

Go to your student's
school website
to obtain information about the 2008 - 09 school year
and any additional site forms that may be required. Not
all schools require additional forms.

6~ Return all necessary forms to your student's school(s) during assigned registration time.
